Power of Attorney for Healthcare (POA): This is a written legal document, drafted by an attorney, that authorizes an individual to make healthcare decisions in the event that you are not mentally capable of doing so.

Power of Attorney for Finance (POA): This is a written legal document, drafted by an attorney, that authorizes an individual to make financial decisions in the event that you are not mentally capable of doing so.

Care Conference: This is a meeting between the individual receiving care or treatment, their case manager, healthcare providers, family, POA, and other patient representatives that discusses and determines the plan for treating or caring for the individual.

Do Not Resuscitate Order (DNR): This is a legal form, completed with a physician, that specifies weather CPR and other life saving interventions are performed in the event of cardiac arrest. This has frequently been replaced by the POLST form.

Physician's Order for Life Sustaining Treatment (POLST):

This is a legal form, completed with a physician, that indicates your individual wishes regarding CPR, life support, invasive, and life sustaining procedures in the event you are unable to communicate. More information about POLST is available on the National POLST Paradigm website.

Living Will: This is a written legal document, drafted by an attorney, that indicates your wishes for healthcare matters in the event that you are not able to do so.

Trust (or Trust Fund): Legal entity established with assistance from attorneys, financial advisers, and accountants that comprises of various assets.
